What to Expect When I Successfully Return Home?

Discharge planning begins when you or your loved one is admitted to our Center and becomes an ongoing review during your treatment.

Your discharge abilities are identified through an assessment of your needs, care plan meetings, and family meetings, to promote safety and independence in your home and community environments.

Below is a list of home needs you may have:

  • Medical equipment needs
  • Finance needs such as Medicaid
  • Home health or other continued therapeutic interventions in the home
  • Outpatient therapists
  • Follow-up outpatient medical appointments

Your health care team will develop discharge instructions specific to your needs for success. Any equipment for your home will be given to you prior to discharge or ordered to be delivered to your home upon arrival.
